What Is a MedsCheck and Who Qualifies in Ontario?

If you take three or more prescription medications, you are entitled to a free, private medication review with your pharmacist — covered by OHIP. It is one of the most valuable services most Ontario patients have never heard of.

What Is the MedsCheck Program?

MedsCheck is a government-funded program in Ontario that pays for a structured, private medication review between a patient and their pharmacist. It was created by the Ontario Ministry of Health in recognition of a well-documented problem: patients taking multiple medications frequently experience preventable adverse effects, unnecessary duplication, dangerous interactions, and confusion about how and when to take their medications. The MedsCheck program aims to close that gap.

During a MedsCheck, your pharmacist reviews every prescription medication, over-the-counter drug, vitamin, and supplement you are taking. They assess whether each medication is still appropriate, whether any interactions exist, and whether you fully understand how to take each medication correctly. The result is a written Medication Action Plan that you receive to keep and share with your healthcare providers.

Who Qualifies for a MedsCheck?

To qualify for a MedsCheck Annual review in Ontario, you must:

  • Have a valid Ontario health card (OHIP)
  • Currently be taking three or more prescription medications for ongoing chronic conditions
  • Not have had a MedsCheck within the past 12 months at any pharmacy

There is no income test and no referral required. The program is available to all qualifying Ontario residents, including seniors, adults with complex chronic conditions, and patients recently discharged from hospital. Types of MedsCheck Services in Ontario

MedsCheck Annual

A comprehensive one-on-one medication review available once per year for patients on three or more chronic medications. Takes approximately 30–45 minutes.

MedsCheck for Diabetes

A specialized review for patients with diabetes, focusing on medications for glucose management, cardiovascular risk, and related conditions. Available annually.

MedsCheck At Home

For patients who are homebound and cannot easily travel to a pharmacy, the pharmacist conducts the review in the patient's home.

MedsCheck Follow-Up

A shorter follow-up appointment (up to 20 minutes) available when significant medication changes occur between annual reviews.

What Happens During a MedsCheck at Tartan Pharmacy?

Your MedsCheck appointment at Tartan Pharmacy is conducted in a private consultation room. Here is exactly what to expect:

  1. 1Bring all your medications — Bring all prescription bottles, OTC medications, vitamins, and supplements to the appointment, or simply bring a written list. The more complete the picture, the more thorough our review.
  2. 2Comprehensive medication review — Our pharmacist goes through each medication: what it is for, how you are taking it, whether the dose is appropriate, and how it interacts with everything else you are taking.
  3. 3Discussion of concerns and symptoms — We will ask about any side effects, symptoms, or changes in health you have noticed. Often, patients describe symptoms that are actually medication side effects they never connected to a specific drug.
  4. 4Medication Action Plan — You receive a written Medication Action Plan — a personalized summary of your medications, any recommendations we have made, and action items for your follow-up with your family doctor.
  5. 5Communication with your prescriber — With your consent, we send a copy of the Medication Action Plan to your family doctor or specialist so they can follow up on any recommendations.
